Lake Houston Physical Therapy, located in Humble, TX is an outpatient physical therapy practice that specializes in orthopedic rehabilitation and sports injuries. Nicole Sontheimer, PT, DPT
Nicole was drawn to the medical field from an early age and initially set her sights on becoming an orthopedic surgeon. While she loved the surgical environment, she found herself wanting a more hands-on role in the full continuum of care, particularly in post-operative recovery. After experiencing an injury of her own and going through physical therapy, she discovered a deep appreciation for the profession and the meaningful impact it has on patients’ lives.
She has been practicing for nearly six years. Her primary concentration is in sports and orthopedic rehabilitation, though she also treats select neurological and pediatric cases. She enjoys working with a wide range of patients, from athletes returning to sport to individuals recovering from injury or surgery. Nicole holds Level 1 and Level 2 certifications in dry needling and is a 200-hour trained and certified yoga instructor.
Physical therapy aligns naturally with her personality and passions. She is detail-oriented and deeply committed to fitness, education, and continual personal and professional growth. The ever-evolving, evidence-based nature of the field keeps her engaged and motivated. She especially values the relationships she builds with patients and reflects on how much she has grown and learned through the privilege of treating others.
Outside the clinic, she comes from a close-knit family that includes her parents and two younger sisters. She became engaged last year to her fiancé of five years, who is a commercial regional airline pilot. They share their home with three cats and are looking forward to starting a family after their upcoming wedding in Wyoming. She loves spending time outdoors and enjoys annual backpacking and ski trips. Fitness is a daily priority, whether through weightlifting, yoga, or training for a half marathon.

Lauren Cruse, PTA
After sustaining an injury during a CrossFit workout, she began physical therapy—an experience that sparked a deep curiosity and lasting appreciation for the profession. Experiencing the recovery process firsthand solidified her desire to further her education and pursue a career as a Physical Therapist Assistant. She is passionate about helping others return to the activities they love, whether that means competitive sports, exercise for overall health, or simply enjoying everyday life with their families.
She graduated in May 2023 and became a licensed Physical Therapist Assistant in July 2023. Her clinical focus includes musculoskeletal injuries, post-surgical rehabilitation, and chronic pain management. She has also completed Dry Needling Level 1 certification.
She believes the foundation of effective physical therapy lies in building meaningful relationships with patients. By understanding each individual’s goals, lifestyle, and challenges, she develops personalized treatment plans designed to support a successful return to daily activities and previous levels of function.
She is happily married to Corey Cruse, and together they have two wonderful daughters, Avaleigh and Adelynn. Their family is completed by their beloved fur baby, Paisley. In her free time, she enjoys spending quality time with her family, embracing her Star Wars fandom, and getting creative with arts and crafts.

FAQ
Yes. The choice of where you receive therapy is always yours. While the majority of our patients come to us by physician referral/recommendation, many clients request to see us due to our strong clinical reputation in the community.
We operate in a direct access state where, if your insurance plan authorizes self-referral to physical therapy, a physician referral is not necessary for you to be seen by a licensed therapist. Please check with your insurance though as many health insurance plans require a physicians referral for therapy to be covered.We also offer treatments, services and/or products that do NOT require a referral and are available on a self-pay basis.
We participate in the majority of major health insurance plans including Medicare and Worker’s Compensation. Please contact us prior to your first visit to see if we participate with your specific health plan.
As a service to our patients, we will gladly file claims with your insurance company on your behalf. In order to appropriately submit these claims, we will need to obtain a copy of your insurance identification card to ensure that we have the necessary information.
Your therapist will conduct an evaluation of your condition lasting between 60-90 minutes. The evaluation is necessary to create a program designed specifically to address your needs for a maximum recovery.
– Referral slip from your medical doctor

– Your ID
– If you are getting therapy as a result of a worker’s compensation claim or an automobile accident, you will also need to bring your claim information, including claim number.
Please arrive 15 minutes early on your first visit to fill out any necessary paperwork.
We recommend comfortable clothing such as sweatpants or shorts, a T-shirt or tank-top and sneakers.
Each visit will last 60 – 90 minutes on average. Times vary due to the treatment and exercises being performed.
The regularity of your visits will be determined after your initial evaluation, but most patients have 3 visits each week scheduled.
